A young athlete complains of knee pain.

There is no dramatic fall, no obvious ligament tear and sometimes not even significant swelling.

The natural reaction is often, “It is probably overuse. Give it a few days.”

That assumption can occasionally be costly.

Osteochondritis dissecans of the knee is an uncommon condition involving the bone just beneath the joint cartilage and, potentially, the cartilage itself.

A small area of subchondral bone can become weakened, and in more advanced cases, the overlying cartilage and bone fragment can become unstable or separate from the surrounding bone.

Think of your knee as having two important layers at the joint surface.

The first is articular cartilage, the smooth tissue that allows the bones to glide with very little friction.

Underneath that cartilage is subchondral bone.

It provides structural support and helps distribute loads through the joint.

In osteochondritis dissecans, a localized area of this underlying bone becomes abnormal.

If the process progresses, the cartilage overlying that area can become compromised.

In severe cases, a section of bone and cartilage may partially or completely detach and become a loose body inside the knee.

Do not keep increasing training load simply because the knee “warms up.”

Pain that repeatedly returns with the same activity is information from your body, not an obstacle to overcome.

2. Swelling after exercise

Swelling can indicate that the knee joint is reacting to irritation inside the joint.

If a young athlete repeatedly develops swelling after running, jumping or sport, I would not treat that as an insignificant training complaint.

Joint swelling combined with persistent pain should prompt clinical assessment.

3. Catching, clicking or locking

This is one of the symptoms I take particularly seriously.

A painless occasional click can be benign.

But catching or locking associated with pain, swelling or restricted movement is different.

If an osteochondral fragment becomes unstable, it can interfere mechanically with normal knee movement.

Patients may describe the knee as catching, getting stuck or suddenly refusing to straighten.

AAOS identifies pain, swelling and mechanical symptoms such as catching, popping and locking among recognized presentations.

4. Loss of full knee extension

A subtle inability to completely straighten the knee can be overlooked.

You may think, “It is just tight.”

But if the knee previously straightened normally and gradually loses extension, particularly when combined with catching or swelling, it deserves evaluation.

A mechanical block is different from simple muscle tightness.

5. Limping

Children frequently adapt their walking pattern without making a major complaint.

A persistent limp can be the body’s attempt to reduce loading through a painful knee.

If your child has started walking differently after sports, investigate the reason rather than repeatedly telling them to “walk normally.”

Initially, OCD-related symptoms may occur only during sport.

As the lesion becomes more symptomatic, pain may begin appearing during everyday activities.

This difference can dramatically change management.

Stable juvenile lesions are often initially treated without surgery.

Unstable lesions, displaced fragments or lesions that fail appropriate conservative care may require surgical consideration.

  • Repeated jumping
  • Sprinting
  • Deep loaded squats
  • High-impact plyometrics
  • Repetitive pivoting
  • Pain-provoking lunges
  • Competitive running
  • Repeated stair workouts when they provoke symptoms

The exact restrictions must come from the treating orthopedic clinician and physiotherapist.

Pain is not a useful trophy in OCD rehabilitation.

Progress should be based on tissue healing and functional criteria, not on how much discomfort you can tolerate.

A lesser-known mistake: returning to sport because pain disappeared

This deserves emphasis.

Pain reduction does not necessarily prove that the underlying lesion has completely healed.

A young athlete may feel better after several weeks away from sport and immediately return to full training.

That sudden jump in loading may be inappropriate.

Current treatment frameworks emphasize lesion stability and healing rather than symptoms alone.

The return should therefore be progressive and medically guided.

When might surgery be necessary?

Surgery is more strongly considered when a lesion is unstable, displaced, persistently symptomatic despite appropriate conservative treatment, or otherwise considered unlikely to heal adequately.

Surgical options depend on the individual lesion.

They may include drilling to stimulate healing, fixation of an osteochondral fragment or procedures to address an osteochondral defect.

The objective is often to preserve the patient’s own joint surface whenever possible, particularly in young patients.

This is one of the most important long-term concepts.

Increase training gradually

Avoid sudden jumps in running mileage, jumping volume or competitive workload.

Respect persistent pain

Pain that repeatedly returns during the same activity deserves attention.

Build recovery into training

Young athletes are often enthusiastic enough to train hard every day.

Recovery is part of training, not the opposite of training.

Strengthen the entire kinetic chain

Appropriate hip, trunk and lower-limb strength can help athletes manage movement demands efficiently, although strengthening cannot guarantee prevention of OCD.

Do not normalize swelling

Swelling after repeated activity is not something to routinely ignore.

Watch movement quality

Persistent limping, avoidance of jumping or altered running mechanics can provide useful early clues.

I recommend professional assessment when knee pain:

  • Persists despite reducing activity
  • Repeatedly returns during sport
  • Causes swelling
  • Produces catching or locking
  • Limits knee extension
  • Causes limping
  • Reduces sports participation
  • Progressively worsens
  • Occurs repeatedly in a growing athlete

Urgent assessment becomes particularly important if the knee becomes locked and cannot move normally, especially when mechanical symptoms are sudden or severe.

What I want parents and athletes to remember

Osteochondritis dissecans of the knee is one of those conditions where the timing of the diagnosis can influence the options available later.

A teenager with recurring knee pain does not necessarily have OCD.

It could be patellofemoral pain, tendinopathy, meniscal pathology, a ligament problem, stress injury or another condition entirely.

But persistent unexplained pain should not be diagnosed as “just overuse” without proper assessment.

And if OCD is suspected, the most important questions are not simply:

“Does it hurt?”

The more useful questions are:

Is the lesion stable?

Is the patient still growing?

Where is the lesion?

How large is it?

Is the cartilage surface intact?

Is it healing?

Those answers guide treatment much better than pain intensity alone.

Final physiotherapy perspective

If you are dealing with persistent knee pain, my biggest advice is simple: do not use exercise as a way to prove that your knee is fine.

Exercise is powerful, but the correct exercise depends on the diagnosis.

With osteochondritis dissecans, a stable lesion in a young athlete may have an opportunity to heal with carefully controlled loading and appropriate medical supervision.

An unstable lesion may require a completely different pathway.

The earlier you identify the problem, the more clearly your healthcare team can protect the joint.

And remember the symptoms that deserve attention: persistent activity-related pain, swelling, limping, loss of motion, catching and locking.

Your knee does not need to be dramatically injured for something important to be happening beneath the cartilage.

Sometimes the earliest warning is simply a young athlete saying, “My knee keeps hurting after practice.”

That sentence is worth listening to.
