“My left leg is 8 mm shorter, so that must be why my back hurts.”
Human bodies are not perfectly symmetrical.
Small differences in leg length are extremely common, and many people have them without experiencing any pain at all.
The real question is whether the difference is large enough, whether your body is compensating for it, and whether those compensations are actually reproducing your symptoms.

Leg length discrepancy, often abbreviated as LLD, means there is a measurable difference between the length of one lower limb and the other.
There are two broad categories.
Structural leg length discrepancy
This means the bones or anatomical structures actually differ in length.
For example:
- One femur may be shorter.
- One tibia may be shorter.
- A previous fracture may have healed with shortening.
- Hip surgery may alter limb length.
- Growth disturbances may produce unequal limb development.
- Certain congenital conditions can create a true difference.
This is sometimes called anatomical leg length discrepancy.
Functional leg length discrepancy
This is more complicated.
The bones may actually be similar in length, but the legs appear or function as though they are different lengths because of factors such as:
- Pelvic position
- Hip mobility restrictions
- Knee positioning
- Foot posture
- Muscle imbalance
- Joint contracture
- Scoliosis
- Previous injury
- Differences in movement strategy
This is one reason I would not recommend measuring your legs with a tape measure at home and immediately ordering a shoe lift.
A 2021 systematic review examining 37 clinical-assessment studies and 15 imaging studies concluded that the standing block test appears to be the most useful clinical method,
while full-length standing radiography is among the most valid approaches for assessing anatomical discrepancy. ((PLOS One))
How Can a Shorter Leg Potentially Trigger Back Pain?
Think about your pelvis as the foundation supporting your spine.
If one leg is genuinely shorter, the pelvis may adapt to the difference when you stand.
The body then has to find a way to keep your head and trunk reasonably balanced over your feet.
That can create a chain of compensations.
A possible compensation pattern
A simplified example looks like this:
Shorter leg → pelvic asymmetry → altered spinal alignment → altered muscle loading → increased mechanical stress → pain in susceptible individuals
But human movement is rarely this straightforward.
Your body may compensate through the:
- Foot
- Ankle
- Knee
- Hip
- Pelvis
- Lumbar spine
- Thoracic spine
And importantly, your nervous system may alter the way you walk to reduce the effect of the discrepancy.
Research examining gait has demonstrated that leg-length discrepancy can produce compensatory changes in the lower limbs and pelvis during walking. ((PubMed))
This is why I look at how you move, rather than simply asking, “How many millimetres shorter is your leg?”
The Pelvis May Be the Missing Link
One of the lesser-known aspects of leg-length discrepancy is that the first noticeable change may occur at the pelvis rather than the lumbar spine.
If one leg is shorter, the pelvis can become relatively lower on that side.
Your body may then make adjustments higher up to keep your trunk upright.
Older radiographic research in people with chronic low back pain found a relationship between leg-length inequality and pelvic tilt,
although the relationship became weaker when researchers followed the alignment higher toward the lumbar spine. ((PMC))
That is clinically important.
It tells us that a pelvic asymmetry does not automatically equal a pathological spinal curve.
In other words:
A difference at the feet does not necessarily mean the entire spine is being damaged.
Your body has multiple ways of absorbing asymmetry.
How Much Leg Length Discrepancy Causes Back Pain?
This is where many online articles become misleading.
You may see claims such as:
- “Anything over 5 mm causes back pain.”
- “A 10 mm discrepancy always needs correction.”
- “You need a lift if your legs differ by 1 cm.”
There is no universally accepted number at which every person develops back pain.
Some reviews and clinical literature consider discrepancies around 10 mm or more potentially clinically significant,
but significance depends on the person, symptoms, biomechanics and context rather than the number alone. (Clinics of Orthopaedic Surgery)
The American Academy of Orthopaedic Surgeons also notes that more than half of people may have some subtle difference in leg length and that mild differences are usually well tolerated. (OrthoInfo)
This is an important point for people who have discovered a small difference on an X-ray.
A measurement is not a diagnosis of the cause of your back pain.
Can a 5 mm Leg Length Difference Cause Back Pain?
Potentially, but you should be cautious about attributing symptoms to such a small difference.
A 5 mm difference may be clinically irrelevant for one person but noticeable to another, particularly when combined with:
- Long periods of standing
- High walking volume
- Running
- Previous hip or knee injury
- Reduced hip mobility
- Significant foot asymmetry
- Repetitive occupational loading
A Finnish occupational study investigating workers with standing and sedentary jobs found an association between leg-length discrepancy and low-back pain,
although the relationship between LLD and back pain remains inconsistent across the wider research literature. (BMC MSK Disorder)
So I would never use “5 mm” as a magic threshold.
The Surprising Part: Your Longer Leg May Hurt More

Here is a lesser-known clinical observation.
Pain does not necessarily occur on the side of the shorter leg.
Biomechanical compensation can distribute forces in ways that make the longer-leg side symptomatic.
An older study involving patients with chronic or recurrent sciatic and unilateral hip symptoms reported symptoms frequently occurring on the longer-limb side. (NCBI)
This is one reason the statement:
“My right leg is shorter, so my right side must hurt”
is not reliable.
Your painful side depends on how your individual body compensates.
Uneven Legs Can Change Your Walking Pattern
Walking is not simply putting one foot in front of the other.
Every step requires precise coordination between:
- Foot pronation and supination
- Ankle movement
- Knee flexion
- Hip rotation
- Pelvic rotation
- Trunk movement
- Spinal stabilization
If leg length is different, the body may modify some of these movements.
For example, someone may subconsciously:
- Alter stride length
- Change pelvic motion
- Spend different amounts of time on each leg
- Modify knee flexion
- Change foot positioning
- Rotate the pelvis differently
- Alter trunk movement
These changes may be completely painless.
But if your tissues have limited capacity to tolerate the additional loading, symptoms can eventually develop.
This is why I prefer the phrase “load redistribution” rather than simply saying that a short leg “twists the spine.”
Can Uneven Leg Length Cause Sciatica?
It can potentially contribute to symptoms that resemble sciatic pain, but it is important not to assume that every leg-length discrepancy-related ache is sciatica.
True sciatica generally involves irritation or compression of a lumbar spinal nerve root and may produce symptoms such as:
- Pain travelling down the leg
- Tingling
- Numbness
- Burning or electric sensations
- Weakness
Leg-length discrepancy can alter pelvic and spinal mechanics, but that does not automatically mean it has compressed a nerve.
If you have neurological symptoms, I would assess the lumbar spine and neurological system rather than simply prescribing a heel lift.
How Do Physiotherapists Check for Uneven Leg Length?
A good assessment should be more sophisticated than measuring from your hip to your ankle once.
I would typically consider:
1. Your history
I want to know:
- When did the back pain start?
- Did it begin after an injury?
- Have you had hip or knee surgery?
- Did you previously fracture a leg?
- Is the pain worse with standing?
- Does walking aggravate it?
- Do you feel uneven?
- Do you limp?
- Have you recently changed footwear or exercise volume?
The history can be surprisingly informative.
2. Standing posture
I may examine:
- Pelvic height
- Iliac crest position
- Knee alignment
- Foot posture
- Trunk position
- Weight distribution
But visual posture alone cannot prove structural LLD.
3. Block test
Small blocks can be placed beneath the apparently shorter limb while you stand.
The clinician observes whether pelvic asymmetry changes as the height is adjusted.
A systematic review found the standing block test to be one of the most useful clinical approaches for assessing LLD. (PLOS)
4. Movement assessment
This is where physiotherapy becomes particularly useful.
I may watch you:
- Walk
- Squat
- Step down
- Stand on one leg
- Bend forward
- Extend your back
- Rotate
- Perform functional tasks
The goal is to understand whether the discrepancy actually changes your movement and symptoms.
Why Measuring Your Legs With a Tape Measure Can Mislead You
This is a very useful fact that many patients never hear.
Clinical measurement methods can have considerable variability.
A systematic review found substantial variation in the validity and reliability of different methods used to measure leg-length discrepancy.
Full-length standing radiography was identified as a strong reference approach for anatomical LLD, while the block test was particularly useful clinically. (PubMed)
Another study comparing clinical measurements found that indirect assessment using lifts performed better than direct tape measurement in detecting discrepancies greater than 5 mm. (Journal of Rheumatology)
Therefore, if your home measurement says:
“My left leg is 7 mm shorter.”
that does not necessarily mean you have a true 7 mm anatomical difference.
Your pelvic position, landmark identification and measurement technique can all affect the result.
What Causes an Uneven Leg Length?
Some people have had the difference since childhood.
Others develop it later.
Common causes include:
Previous fractures
A femur or tibia that heals with shortening can create a genuine structural discrepancy.
Hip replacement
Changes in limb length can occur following hip arthroplasty.
Knee surgery
Joint replacement or other procedures can alter the functional relationship between the lower limb segments.
Growth disturbances
Conditions affecting bone growth can result in one limb becoming longer than the other.
Hip or knee contractures
Even when bone length is equal, an inability to fully extend a joint can create a functional difference.
Foot and ankle mechanics
Changes in foot posture can alter functional limb length during weight bearing.
Pelvic asymmetry
Pelvic position may make one limb appear shorter without an equivalent difference in bone length.
This distinction between structural and functional LLD is one of the most important concepts when treating back pain associated with leg-length inequality.
Can a Shoe Lift Fix Back Pain?
Sometimes.
But it should not be treated as a universal solution.
A systematic review of adults with LLD and musculoskeletal conditions found that shoe lifts were associated with pain reduction and improved function in several studies.
However, the authors emphasized that the evidence quality was generally low and that lift prescription methods were inconsistently described. (Elsevier)
A small clinical study involving people with chronic low back pain and measurable LLD also reported improvements in pain and disability after individualized lift intervention. (JOSPT)
This is encouraging, but it does not mean:
“Buy a heel lift today.”
The amount of correction matters.
The adaptation period matters.
The underlying cause matters.
And your symptoms matter.
Should You Correct the Entire Leg Length Difference?
Not necessarily.
This is one of the most important points I would make as a physiotherapist.
If your measured difference is 12 mm, it does not automatically mean you should immediately put a 12 mm lift under one foot.
Your body may already have developed successful adaptations.
Suddenly changing the mechanics can feel strange or uncomfortable.
The appropriate correction, if correction is indicated, should be individualized and monitored.
The goal is not to make your skeleton look perfectly symmetrical on paper.
The goal is to improve function and reduce symptoms without creating new problems.
What Physiotherapy Can Do for Leg-Length-Related Back Pain
Physiotherapy is not simply about giving you exercises.
The first goal is identifying whether the discrepancy is actually relevant.
Treatment may include:
Hip mobility work
If hip stiffness is forcing your pelvis to compensate, improving hip movement may reduce mechanical stress.
Gluteal strengthening
Improving hip strength can help your pelvis remain controlled during single-leg loading.
Trunk endurance training
Rather than obsessing over “core activation,” I prefer progressive trunk endurance and functional strengthening.
Foot and ankle assessment
Your foot may be contributing to the apparent discrepancy.
Gait retraining
If your walking pattern has become highly asymmetrical, retraining may be useful.
Gradual lift intervention
When a genuine structural discrepancy appears clinically relevant, a physiotherapist or appropriately trained clinician may recommend a carefully selected lift.
Load management
This is often overlooked.
If symptoms appeared after suddenly increasing walking, running or standing time, the discrepancy may be only one part of the problem.
A Lesser-Known Tip: Test the Hypothesis
If I suspect your leg-length discrepancy is contributing to your symptoms, I want to test the hypothesis, not simply assume it.
For example, under professional supervision, a temporary lift may be trialled while observing:
- Pelvic position
- Walking
- Pain response
- Standing tolerance
- Functional movement
If your symptoms consistently improve with an appropriately selected intervention, that gives us useful clinical information.
If symptoms become worse, that is also information.
This is much more sensible than permanently modifying your shoes based on a single measurement.
What Should You Avoid?
Do not buy a large heel lift immediately
A bigger lift is not necessarily better.
Do not assume every back problem is caused by leg length
Disc problems, facet-related pain, sacroiliac-region pain, muscular pain and many other conditions can produce similar symptoms.
Do not repeatedly measure your legs at home
Small differences between measurements may simply reflect measurement error.
Do not try to “straighten” your pelvis all day
Your body is designed to move, not maintain one perfectly symmetrical posture.
Do not stop exercising
Unless a particular activity clearly aggravates your symptoms, excessive rest can reduce your physical capacity.
Do not ignore neurological symptoms
Numbness, progressive weakness or significant radiating pain deserves proper assessment.
When Should You See a Physiotherapist?
Consider an assessment if you have:
- Persistent low back pain
- Recurrent one-sided back pain
- Pain that increases after prolonged standing
- A noticeable limp
- A history of lower-limb fracture
- Previous hip or knee replacement
- A diagnosed leg-length discrepancy
- Recurrent symptoms despite conventional back exercises
- Significant asymmetry during walking
- Hip or knee symptoms occurring alongside your back pain
A physiotherapist can help determine whether the discrepancy is actually contributing to your symptoms or simply an incidental finding.
Red Flags: When Back Pain Needs Medical Assessment
Do not assume that leg-length discrepancy explains your symptoms if you develop:
- New bladder or bowel control problems
- Numbness around the groin or saddle region
- Rapidly worsening leg weakness
- Severe neurological symptoms
- Major trauma
- Fever with severe back pain
- Unexplained weight loss
- History of cancer with new unexplained back pain
- Severe, unrelenting night pain
These symptoms require medical evaluation rather than a shoe lift experiment.
Can Uneven Leg Length Be Prevented?
Congenital or anatomical differences cannot necessarily be prevented.
However, you can reduce the likelihood that an existing discrepancy becomes problematic by maintaining good physical capacity.
Focus on:
Keep your hips strong
Strong hip muscles help control the pelvis during walking and single-leg activities.
Maintain ankle mobility
Restricted ankle movement can change lower-limb mechanics.
Train both sides
Do not always favour your stronger leg during exercise.
Progress walking and running gradually
A previously tolerated discrepancy may become symptomatic after a sudden increase in training volume.
Replace badly worn footwear
Unevenly worn shoes can introduce another source of asymmetry.
Maintain healthy body weight
Higher mechanical loads can increase the overall demand placed on the spine and lower limbs, although body weight alone does not determine whether LLD will become painful.
One More Important Point: Your Shoes Can Hide the Real Problem
Imagine someone has a 9 mm anatomical discrepancy but also has:
- Limited ankle dorsiflexion
- Weak hip abductors
- Poor single-leg control
- Reduced hip extension
- A sedentary lifestyle
- A sudden increase in walking
Which factor is causing the pain?
It may not be possible to answer that from a leg-length measurement alone.
This is why I consider leg-length discrepancy a potential contributor rather than a diagnosis of back pain.
What Does the Research Actually Tell Us?
The research is more nuanced than many online health articles suggest.
Older radiographic studies demonstrated relationships between leg-length inequality, pelvic tilt and spinal alignment. (PubMed)

Your body is remarkably good at adapting to asymmetry.
The more important question is not:
“How many millimetres shorter is my leg?”
It is:
“Is this difference changing the way I move, and is that movement contributing to my symptoms?”
That is the question I would want to answer before recommending exercises, orthotics or a shoe lift.
